I have just played a game where a guard which I had doubled was injured on
the way through to the main character. This may have happened anyway but it
was a good guard and I was trying with a not particularly good agent. This
suggests that double agents can allow themselves to be hurt.

In my experience, no. Self defense trumps treachery in this case.

Makes sense, you would at least want to make it look like you were
doing your job in order not to have your cover blown. Look at Kim
Philby.

Sounds like in operations involving the nation for whom he is "working" his
skill is reduced. Did they ever compile this type of data in the Mouth?
Simply cut the skill in half or so if the character/pop in question is of
nation X. Thus, he'll fail orders more often and results of guarding will
be less stellar than his real levels would lead you to expect. For every
rationalization, there has to be an algorithm programmed into a relatively
simple computer that makes some sort of sense, no?
