Can you challenge a hostage? I'm guessing 90% NO,
with the pdf saying that "so and so unavailable" or
something like that?

I agree with the rulebook in this case. If you hold an enemy hostage, and therefore presumably unarmed - you've certainly taken any artifacts from him - , you cannot (fairly!) challenge him.
If you want to kill the hostage, you should execute him.
Where I disagree with the rulebook is, why is 650 Execute an agent order? What is difficult or specialist about it, that it requires agent skill? Any idiot with a blade can kill an unarmed prisoner.
